Family Foundation Center ensures a smooth enrollment process for our participants by providing their caregivers with a comprehensive packet of information. This packet must be completed before commencing services with Family Foundation Center. All the information requested within the packet is essential to secure prior authorization for funding, which enables us to initiate the participant’s assessment and services. FIRST STEPS

Before applying for enrollment in our ABA program at Family Foundation Center, please carefully review the following essential enrollment criteria:

  1. The child must have received a medical diagnosis of Autism Spectrum Disorder within the past year.
  2. Families must maintain active insurance coverage that includes Autism and/or ABA, EIBI, or EIDBI therapies. Please refer to the list of accepted insurance providers provided below for further details.
  3. A minimum commitment of six months is required for attendance at Family Foundation Center.
  4. Parental involvement is mandatory, including but not limited to attending parent meetings, participating in monthly family skills training sessions, joining quarterly treatment plan meetings, and engaging in Parent Resource Groups.
  5. Parents are responsible for arranging transportation for their child to and from the center. Additional information on transportation options is available here.
  6. While at the center, the child must adhere to a gluten/casein/peanut/dye-free diet.

OUR INTAKE PROCESS

Please follow these steps to apply for enrollment at Family Foundation Center:

  1. Download and fill out our application form. Please ensure all required medical documents and insurance cards are included.
  2. Once we receive your complete documentation, our team will review it thoroughly.
  3. We will verify insurance information directly with your provider.
  4. Expect a call from our Intake Coordinator to arrange an Intake Interview with our Clinical Team and a tour of our facilities at Family Foundation Center.
  5. Following the intake interview, the Clinical Team will provide a recommendation. If approved, your child will be added to our waitlist.
